Part-Time Academic Coach, Harcum College

Harcum College in Bryn Mawr, PA, a leader among the nation’s independent, two year colleges, has an opening for a Part-Time Academic Coach for the Student Support Services (SSS) Program during the academic year.  Under the direction of the SSS Programming Director, the Academic Coach will work directly with students to increase their academic success by providing skill-based services and academic coaching.

Responsibilities

  • Provide: Academic Coaching/Time Management, Individual Academic Planning, Soft Skill/Interpersonal Training, Financial Literacy Training, Psychosocial Support and Individualized Interventions.
  • Enroll, assess, and plan each student’s Individual Academic Plan – assessing non-cognitive skills and barriers that might negatively impacts a student’s academic success.
  • Coach new and continuing students. Schedule bi-weekly meetings with a case load between 40-50 students.
  • Monitor and document each student session. Follow each student’s academic progress throughout the academic year.
  • Refer students to various departments/services: career and transfer, tutoring, and counseling.
  • Schedule and facilitate student focused workshops/sessions: educational, cultural, and academic.
  • Maintain cooperative relationships with students, faculty and staff. Work collaboratively with the Educational Success Center and be a representative for Student Support Services and the Student Success Center.
  • Attend program meetings and provide assistance and advice to increase student participation to SSS programs.

Qualifications

  • Education: Bachelor’s degree (minimum) Master’s degree preferred.
  • Experience: Experience in higher-education, counseling, coaching, or with Federal/State funded programs similar to TRiO, excellent written and verbal communication and computer skills.
  • Physical: The position requires the ability to bend, lift, climb, stairs, and carry up to 30 pounds.
  • Schedule: This is a part-time position (fifteen or twenty hours a week) during the academic year, about 10 months out of the year. This position will be based on the Bryn Mawr campus within the traditional work hours. There is a possibility to work during summer sessions.

Student Support Services is a federally-funded TRiO program to support students by overcoming educational, cultural, social, and economic barriers to higher education. SSS provides services to students such as but not limited to: tutoring, academic coaching, educational/cultural programming, and financial literacy training.  SSS provides individual services to students who are first-generation, income eligible, or have a disability.

Trans-Health Information Project (TIP) Co-Coordinator/Outreach Specialist, GALAEI

Position Summary:
The TIP Co-Coordinator/Outreach Specialist is responsible for connecting and building relationships with outside agencies and programs and to identify where and when outreach is to be conducted, including, but not limited to providing HIV risk-reduction services to HIV+ and HIV-negative transgender, gender non-conforming, and gender variant (trans*) individuals. The TIP Co-Coordinator/Outreach Specialist is responsible for ensuring program deliverables are met, including recruitment, counseling, and referrals. The TIP Co-Coordinator/Outreach Specialist works as part of a team to implement social justice services for Trans* communities.

MAJOR RESPONSIBILITIES:

  • Provide individual counseling to trans* individuals on various sexual health issues such as HIV testing, HIV prevention, HIV care if infected, healthy sexual behavior, transitioning, etc.;
  • Provide sexual health counseling designed to assist Trans* individuals and their families deal with the psychosocial, emotional, financial and environmental issues of HIV;
  • Develop and implement outreach plans in partnership with the Executive Director and other Co-Coordinator to bring in transgender, gender non-conforming and gender variant individuals. This includes building relationships with other agencies/organizations as well as individual outreach;
  • Respond to clinician referrals and clients request for case management assessment and intervention within required response time;  Perform initial comprehensive sexual health counseling assessment and develop a plan of intervention that addresses identified need for crisis intervention, brief solution focused or grief and bereavement counseling, continuity of care planning and coordination of needed community services;
  • Report weekly on program data, evaluation web and internal data collection protocol;
  • Provide patient and family education on psychosocial and adaptive aspect of the disease processes and offer education on accessing relevant services and resources;
  • Provide advocacy services to ensure linkage to appropriate services or resources for patients who are eligible for such;
  • Participate in interdisciplinary care planning and collaboration to ensure effective and efficient coordination of inpatient and ambulatory care resources;
  • Perform all administrative tasks related to caseload such as medical record documentation, entitlement applications, departmental statistics, continuity of care referral paperwork, team reports, etc.;
  • Participate in organization grant related clinical and administrative meetings and activities;
  • Demonstrate the knowledge and skill necessary to provide care based on psychosocial, educational, safety and related criteria appropriate to the age of clients served in assigned service area;

HIV/HCV Linkage Supervisor

The Philadelphia Department of Public Health (PDPH), AIDS Actvities Coordinating Office (AACO) has been awarded funding to improve the continuum of hepatitis C care for people who are living with HIV and also at risk for or living with hepatitis C (HCV). This three year project includes field activities to locate and provide assistance to HIV/HCV positive persons in accessing services along the hepatitis C continuum including testing, follow up care, and treatment.

In Year One, the HIV/HCV Linkage Supervisor will be responsible for the development of HCV Linkage Protocol and coordination of activities related to hepatitis C linkage to care. Overseeing a team of two Linkage Coordinators, the HCV Linkage Team will begin supporting clients in the Ryan White Care system to improve hepatitis C outcomes. In years two and three, the HIV/HCV Linkage Team will also help identify opportunities to integrate hepatitis C linkage support in other AACO funded programs, such as Medical Care Management, and build capacity among the existing HIV service structure to sustain patient access to hepatitis C services in addition to HIV services.

Daily Job Responsibilities: 

  1. Conduct strengths and needs assessments, outreach for potential participants.
  2. Engage, educate, activate and support volunteer peers to deliver peer recovery support services for individuals and families seeking to access and sustain recovery throughout Chester County.
  3. Develop, in conjunction with staff, orientation and training program for volunteers; develop and nurture peer leadership skills in members of the recovery community.
  4. Provide volunteers with job descriptions, manuals and handbooks for volunteer programs.
  5. Inform volunteers regarding trainings, workshops and pertinent educational opportunities.
  6. Assure proper supervision of volunteers; maintain volunteer records and schedules; interpret Council policies for the volunteers. Follow proper protocols and procedures regarding volunteers and volunteer coordination.
  7. Work with Director of Outcomes and Evaluation to support evaluation component of this project.  Ensure GPRA process and reports are completed in a timely manner.  Maintain project logs, reports and records, data input.
  8. Promote programs and services both internally and externally to ensure successful programming, events and strong volunteer base.  Identify community resources for volunteer recruitment & marketing of programs and services.  Maintain an understanding of, and adapt to, individual community dynamics.
  9. Collaborate with intersecting systems to provide a coordinated approach to recovery support services, both telephonic and non-telephonic.  Work collaboratively with Council staff and volunteers to develop and implement the goals and objectives for this project.
  10. Provide a comprehensive array of programs and activities to support recovery for individuals and families, following the goals and objectives defined in the proposal supporting this project.
  11. Work with Peer Leadership Councils and Chester County PRO-ACT Board and community agencies in Chester County to ensure vibrant, active collaboration.  Representation should include multiple pathways to recovery, family members and young people in recovery.
  12. Serve as liaison and support to/for all PRO-ACT Chapter committees.
  13. Develop a recognition process for volunteers; maintain a good level of volunteer morale.
  14. Assist with any additional tasks assigned by the Peer Leader Coordinator or Executive Director.

Case Management and Adult Programming Services Supervisor, Lutheran Settlement House

Mission:

Established in 1902, Lutheran Settlement House (LSH) is a non-profit, community-based organization committed to serving vulnerable children, adults, and families living in Philadelphia. Over the past century, the programs and services offered by LSH have changed in response to the evolving needs of the community. Scope of Position:  

The Case Management and Adult Programming Services Supervisor ensures case management compliance with all contract requirements. S/He serves as the adult programming and case management liaison between all LSH departments and their funders and ensures the provision of clinically appropriate, trauma-informed social work services to all clients.

Functions and Duties:

  • Reflects in professional practice the mission and philosophy of Lutheran Settlement House.
  • Provides weekly trauma-informed supervision to family advocates and manages caseload assignments.
  • Supervises family advocates in the evaluation of services. Monitors client progress toward established goals and objectives.
  • Manages elective client savings. Ensures safe collection of resident savings, maintenance of accurate records, and completion of appropriate documentation related to savings and payments.
  • Provides on-going support and monitors provision of mental health services through FaSST provider.
  • Ensures timely and accurate entry of Client Track data.
  • Intervenes during periods of client crisis to provide direction and support to family advocates and shelter staff.

  • Maintains collaborative relationships with community service providers including Department of Human Services (DHS) mental health and substance use providers.
  • Develops and maintains contact with other agency staff to develop specialized services and programming for mothers in residence.
  • Serves as liaison between Jane Addams Place and other LSH programs in order to provide on-going adult programming and linkages to cross-agency services.
  • Supervises social work students in the completion of daily tasks, interactions with residents and implementation of case plans.

Qualifications:

  • Master’s degree in social work or human services related field required.
  • Experience working with homeless children and/or families is preferred.
  • Prior supervisory experience is preferred.
  • Demonstrated clinical social work and administrative supervisory skills.

  • Comprehensive knowledge of trauma informed care.
