THE ORGANIZATION

POSITION SUMMARY

The Care Manager is responsible for assisting those we serve in achieving their goals. We organize these goals under 6 major Life Ares (Benefits, Housing, Vocational, Health & Wellness, Personal Finance and Social Connection). Care Managers work as a collaborative team to achieve goals that positively impact the lives of those we support. While we do work as a team, we also need team members who are able to independently to engage the person, their support system and the human services community to assist the person in achieving the identified goals. While we are always available for collaboration, questions and support, a good level of autonomy will ultimately be expected as you grow into the position and become most familiar with those you serve.

Some of the people we support pay for services privately while others have a Special Needs Trust that can be utilized for the sole benefit, often financing Care Management and other much needed supplemental resources. This position requires travel within the Greater Philadelphia region (Bucks, Chester, Delaware, Montgomery, and Philadelphia counties).

Position Role & Outcomes:

  • Initial plan development and ongoing annual plan review with the person served and his/her support system.
  • Provide consultative review to direct families towards benefits, resources and services that will help their family member with a special need live a more meaningful life.
  • Work with the client on a continual basis in accordance with the guidelines and objectives of the PLAN service contract.
  • Utilize all available resources; private and county, government (etc.) to support the person served as necessary to maximize his/her well-being. (Including identification and referral to appropriate resources; crisis management; assistance with daily living activities; secure housing, public benefits, necessary medical care, home visits or transporting to and from appointments.)
  • Demonstrate capacity to assist with public benefits acquisition.
  • Serve as client advocate with support of PLAN of PA team.
  • Interact effectively within the PLAN of PA team to ensure that the financial and other support needs of the client are addressed.
  • Accurately and timely complete all of your own client engagement, documentation, billing and expenses.
  • Review and approve monthly billing to assure accuracy.
  • Collaborate with the PLAN of PA team to assist in improving family dynamics and addressing legal issues.
  • Attend continuing education classes or other trainings to continually develop industry and clinical knowledge.
  • Utilize effective problem solving skills to ensure the best/optimal outcome for the client.
  • Assist client in establishing and following a realistic monthly budget.
  • Other duties as assigned.

Qualifications:

  • Pursuing a master’s degree; MSW, LCSW strongly preferred.
  • Minimum of 2 years Case Management experience in mental health related field.
  • Excellent planning, organizational and interpersonal skills.
  • Some experience of government benefits, Social Security Administration, Waiver Program, Medicare, Medicaid, Section 8 Housing, SNAP, etc.
  • Excellent communication and writing skills.
  • Experience with Microsoft Office products and other software.
  • General understanding of budgeting and money management.
  • Valid PA driver license, valid car inspection and proof of insurance.
  • Clear criminal record and child abuse clearance

Part-time Home Visiting Parent Coach, Attachment & Biobehavioral Catch-up Lab, Department of Psychological & Brain Sciences, University of Delaware

Hiring: Part-time home visiting parent coach willing to visit homes in Delaware
County, PA and Philadelphia.

  • This is a part-time, miscellaneous wage position with no university benefits.
  • Hours will vary based on caseload and parent coach’s interest; average 3-6 hours a week.
  • Commitment of one year requested.
  • Parent coach will be compensated for travel time and reimbursed for mileage.
  • Compensation is competitive and based on education and experience.

Responsibilities

  • Parent coach will be responsible for delivering two home-visiting interventions:
    • Safe Environment for Every Kid (SEEK) – a brief case management intervention (1-2 visits) connecting families to local resources.
    • Modified Developmental Education for Families (mDEF) – a 14-session
      parenting intervention focused on teaching parents activities that stimulate their infants’ cognitive and motor development.
  • Intervention sessions to be scheduled at the parent coach and mothers’ convenience (may include evenings and weekends).
  • Clientele will be pregnant and peripartum mothers in treatment for opioid dependence and enrolled in a randomized clinical trial.
  • Parent coach will participate in biweekly supervision with a licensed clinical social

Education, Experience, and Abilities

  • Requires a Bachelor’s degree, ideally in a field related to human services.
  • Prior experience working with parents and infants preferred.
  • Our ideal applicant is organized and responsible, with good verbal communication skills and a warm but professional demeanor.

FT Program Coordinator, The Penn Memory Center

Behavioral Health Coordinator, Community Volunteers in Medicine

Position Summary

The Behavioral Health Coordinator’s primary responsibilities include
Spanish counseling of CVIM patients, as well as coordination of all Behavioral Health Services for CVIM. This includes management of the team of Counselors and Psychiatrists and the referral process for patients within CVIM and those referred to outside agencies.

Major Responsibilities:
• Provides Spanish counseling to CVIM patient referrals

Full-time Social Worker, Mercy LIFE West Philadelphia (Living Independently for Elders, a Trinity PACE Program)

Responsible for all social work of the LIFE program. Works in collaboration with the Manager, as well as the Interdisciplinary Team(s), in establishing new systems and procedures to standardize and coordinate operations. Takes the lead role to clarify, exemplify, and maintain standards and policies for social work practice in LIFE.

Community Intervention Specialist/Social Worker, The Center for Nonviolence and Social Justice/Healing Hurt People

Position requires a detail-oriented, organized, self-starter for the provision of acute intervention and follow-up services based out of Hahnemann Hospital and other local hospitals and provided by the Healing Hurt People Program (HHP). HHP is a trauma informed, hospital based, community focused intervention strategy dedicated to reducing violent injuries, deaths, and the long term consequences of trauma and to increasing physical and emotional healing amongst young people in Philadelphia. HHP has moved to a fee-for-service model, and the position will require a flexibility and willingness to meet the requirements of this transition.

The Community Intervention Specialist will provide acute intervention, outreach/advocacy, counseling, and case management for victims of interpersonal violence between the ages of 18 and 35. The position requires the ability to provide clinical services in a non-traditional manner/setting, utilizing a wide range of strategies to address symptoms of trauma and disruptions in daily living (safety, emotional management, loss, and future). The candidate must also be able to provide case management services, and to support participants in the program in increasing connection to school, employment, family, social supports, and social services. Job requires a significant amount of community/home based work, and a willingness to engage within a diverse range of neighborhoods in Philadelphia.

Qualifications:

  • Masters level degree in Social Work or related license eligible profession required
  • Must already be licensed or able to become licensed immediately upon hire
  • Reliable personal transportation required, car strongly preferred
  • Experience providing services in a fee-for-service or billing environment strongly preferred
  • Ability to complete the billing requirements including maintenance of timely and accurate chart records (including but not limited to intakes, treatment plans, notes, signatures) required
  •  Demonstrated ability and desire to offer marginalized individuals a high quality of service
  •  Documented history of serving at-risk populations including homeless individuals, communities in poverty, at-risk youth, individuals impacted by trauma/adversity, and/or justice-involved youth or adults
  •  Strong interpersonal, communication, organizational and writing skills are essential
  •  Ability to motivate and establish therapeutic alliance with clients
  •  Knowledge of, and demonstrated skills in, clinical counseling and interventions
  •  Experience performing social service case management a plus
  • Computer literacy required (ability to compose and send emails, draft client case notes, use an internet-based database system, compose documents in Microsoft Word, and perform basic internet research)
  •  Ability to work independently and as part of a team
  •  Ability to multi-task and effectively manage work at different sites (in-office and off-site)

Essential Functions:

  • Provide trauma informed emotional support, mentoring, and advocacy to violence-involved individuals (who are seen as a result of community violence) within the hospital, in home, and in the community
  • Provide referrals and intensive case management to assist in successful connection to community based resources
  • Maintain intensive follow-up contact with clients, family, friends and service providers through home visits and telephone contact
  • Utilize trauma informed modalities and interventions with clients, including but not limited to Motivational Interviewing, Trauma Art Narrative Therapy, TF-CBT, the SELF model
  • Co-facilitate SELF groups
  • Keep consistent and accurate records of all contacts with clients in web-based database, provide documentation needed for billing for services
  • Disseminate information and resources to hospital staff, patients, community groups, local leaders and the general public
  • Other duties as assigned
