Position Summary
Join a vibrant campus community whose excellence is reflected in its diversity and student success. West Chester University of Pennsylvania’s Division of Student Affairs invites applications for the position of Case Manager for CARE Support Services. This is a one-year temporary appointment with the possibility of extension.
Under the supervision of the Assistant Vice President for Identity, Health, and Wellness, Students, the Case Manager for CARE Support Services provides non-clinical case management services to students with at-risk mental health and social support needs through interventions, referrals, and connection to follow-up services. The Case Manager will be an advocate for student success and possess a demonstrated ability to multitask and manage a diverse caseload.  The Case Manager will develop and maintain a database of referral resources and community services.  The Case Manager will document and track cases using established protocols. In addition to direct service, this position coordinates with community resources and campus partners including Academic Affairs, Dean of Students Office, Residence Life, Counseling Center, Office of Services for Students with Disabilities, and Registrar. The Case Manager will also serve as member of the CARE Team. This is a 12-month full-time position.
 

Responsibilities:
Case Management and Student Support (60%)

  • Serves as a case manager for students experiencing emerging or chronic situations that may compromise their ability to be successful.
  • Meets with individual students to assess needs.
  • Implements non-clinical interventions, programs, and/or services.
  • Coaches students towards strategies and interventions that support self-care and self-advocacy.
  • Provides referrals to campus and community resources.
  • Monitors and documents student progress and adherence to established individualized care plans.
  • Maintains accurate and confidential student records.

CARE Team Support (10%)

  • Attends CARE team meetings.
  • Provides support to the CARE Team, which includes setting meeting agendas and updating files.

Administrative Support (15%)

  • Responds to CARE reports received by the end of each business day.
  • Maintains directory of campus resources available to students.
  • Maintains websites and marketing materials.
  • Assists with coordinating assessment, evaluation and research related to CARE support services.
  • Prepares departmental reports as needed.
  • Supports the office of Student Assistance when needed.

Event/Program Support and Coordination (10%)

  • Provides CARE-related event support to which includes presentations and event tabling.

Professional Development (5%)

  • Attends professional development opportunities, including monthly Student Affairs meetings/conferences.
  • Attends professional development opportunities related to case management or behavioral intervention teams.
Minimum Qualifications
  • Master’s degree in counseling, social work, higher education/student affairs, or related areas of study.
Preferred Qualifications
  • Experience in crisis intervention
  • Experience working as part of a case management support team
  • Minimum two years full-time experience in case management
  • Minimum two years of full-time experience in higher education
Special Instructions
The salary is $48,118 annually. Excellent benefits package including tuition fee waiver for self and dependents. Applicants must successfully complete interview process to be considered as a finalist.
